A LEGAL GUARDIAN MUST BE PRESENT FOR A PATIENT’S FIRST VISIT TO OUR OFFICE.

If someone other than a legal guardian will be bringing your child/ren to future appointments, please make a notation in Section 6 of the “New Patient Packet” or complete an “Authorization for a Caretaker” form.

We are committed to providing the highest quality pediatric care for you and your family. We strive to see all patients on time for their scheduled appointment. Occasionally our schedule is delayed to accommodate an anxious child or an emergency. Should this occur, please accept our apologies in advance. We promise to give the same courtesy to your child if he or she needs extra care.

The scheduled appointment is reserved specifically for your child. Any change in this appointment affects many patients. If rescheduling an appointment is unavoidable, please call at least two business days in advance.
